Waiver for Teams

TEAM / COACH / ORGANIZATION REPRESENTATIVE CERTIFICATION (ADMIN CONVENIENCE — NOT A SUBSTITUTE)

Important: This Certification does not replace the requirement that each adult Participant (18+) sign the waiver themselves and each minor Participant have a parent/legal guardian sign the waiver. This Certification is for streamlined check-in and verification only.

I (“Team Representative”), certify as follows regarding my team/organization:

  1. I confirm that each and every Participant on my roster has executed a valid On Deck Cages waiver as follows:

    • For Participants age 18+, the Participant signed their own waiver; and

    • For Participants under 18, the Participant’s parent/legal guardian signed the waiver.

  2. I understand that I am not signing the waiver on behalf of other adults or minors unless I have separate written legal authority to do so, and that On Deck may require individual waivers at any time.

  3. I agree to promptly provide copies of all executed waivers to On Deck upon request.

  4. To the fullest extent permitted by law, I agree to indemnify and hold harmless the Released Parties from claims arising from any misrepresentation in this Certification or failure to obtain required signatures, except where prohibited by law.
